Below is a list of items (*) provided by the school system to all clinics and used regularly for first aid. Should you know of a reason why these items should not be used on your son/daughter, please contact the school nurse at 658-6320 ext 107.

New Students

A new preschooler, kindergarten student, or first through fifth grader entering a public school for the first time must furnish a physical examination /School Health Entrance Form (MCH 213) performed no earlier than twelve months prior to the date of attending school. This physical exam is to include all components as stated on the MCH213. A signed Certificate of Immunization (part 2 of MCH213) must be presented prior to entry. Students in grades 6-12 do not need to provide physicals for school entry unless they are coming from outside the United States, only the Certificate of Immunization.   A TB Risk Assessment must also be completed at the time of registration for all new students. No child may be admitted to school without the required components for his school level.

Your child must be immunized against Diphtheria, Pertussis (whooping cough), Tetanus (DPT), Poliomyelitis (IPV/OPV), Measles (Rubeola), Mumps, Rubella (MMR), Hepatitis B, and Chicken Pox (Varicella).

The following are required of all students:

-Four doses of DTaP, with one dose after the fourth birthday; for all new students as of fall 2010

-Four doses of IPV/OPV, with one dose after the fourth birthday; for all new students as of fall 2010

-Two doses of measles and mumps vaccine, and one dose of rubella vaccine (usually given as        MMR) following the recommended schedule beginning fall 2010

- Three doses of Hepatitis B following the recommended schedule

-Two doses of varicella vaccine for students entering kindergarten as of fall 2010, One dose Varicella vaccine for students grade 2-9

-A Tdap (Diphtheria, Pertussis, and Tetanus) booster is required of students grades 7-12 as of Fall 2019

-An HPV(Human Pappillomavirus) vaccine is recommended for girls entering 6th grade as of fall 2009

Immunization records must show the month, day, and year of vaccines received as well as signature of health care provider. 

Should you need assistance in obtaining the necessary immunizations for your child and/or health records, please call your child’s physician or the Stafford County Health Department at (540) 659-3101. The Stafford County Health Department offers free immunizations every Friday from 8:00 a.m. to 12:00 p.m.
