Use the forms below to authorize or change deductions from your paycheck for various purposes, including credit union allotments.

Submission Instructions

To start or change a deduction:

1. Download and complete the relevant form(s) below. 
2. Email the completed form to Juliette Klocek at klocekjm@staffordschools.net in the Department of Payroll & Benefits, or interoffice the form to her attention.
